This version simplifies and corrects the ABNF in section 4.1.1, and
removes the recommendation to suppress duplicates before reporting
statistics in 4.4.2, since it conflicts with the definition of the
Lost Packets Counter, which is done in reference to RFC 3550 and so
cannot easily be changed. In addition, there are assorted editorial
fixes and clarifications throughout.

This version adds Ken Carlberg and Piers O'Hanlon as co-authors, and
merges some ideas from
draft-carlberg-avt-rtp-ecn-02. The mechanism has been updated to
more clearly separate the information we believe is necessary to report
in RTCP for ECN to work in an RTP context, and that needed for the ECN
Nonce. The RTCP reports have also been updated to use different formats
for regular RTCP reports, and early/immediate feedback sent in response
to an ECN-CE mark.
